      The 2016 revision to the World Health Organization classification of myeloid neoplasms and acute leukemia comprises two stages of primary myelofibrosis (PMF),early/prefibrotic primary myelofibrosis (Pre-PMF) and overt fibrotic PMF (Overt PMF).It was diagnosed based on histopathological features of bone marrow biopsy specimens,fibrosis grade and clinical parameters [leukocytosis, anemia,elevated lactate dehydrogenase values, and splenomegaly].In addition, accurate differential diagnosis is essential for Pre-PMF.Accurate differentiation from essential thrombocythemia (ET) is a key issue as the two entities show different clinical presentation and outcome, in terms of survival, leukemic evolution and rates of progression to Overt PMF.The current review provides an overview on how to diagnose patients with pre-PMF and compare clinical 、morphology、genetic features and prognosis between the two entities.
